If your condition may require additional care or you need a DOT for FAA exam, I suggest that you arrange to place yourself under the care of another doctor. To assist you in receiving the care that you need, I will be happy to provide your new doctor with copies of the necessary records from your file.
Please contact my office via firstname.lastname@example.org to sign an authorization to send your records, or to obtain the mailing address to send an authorization to. There will be a charge based on the number of pages of copies of your record.
