Because I have just recently accepted a new position out of state, I have closed my office at 108 Avon Avenue, Washington, NC 27889 and 845 Johns Hopkins Drive, 27834 on 11/30/2020. I will not be available to attend to you after that date.

If your condition may require additional care or you need a DOT for FAA exam, I suggest that you arrange to place yourself under the care of another doctor. To assist you in receiving the care that you need, I will be happy to provide your new doctor with copies of the necessary records from your file.

Please contact my office via [email protected] to sign an authorization to send your records, or to obtain the mailing address to send an authorization to. There will be a charge based on the number of pages of copies of your record.

I regret that I will not be able to continue to serve you. My years here have been filled with many rewarding experiences and memorable patients.
